This form is the only PROOF that we have that an applicant to our distance program actually has a PRECEPTOR who has agreed to precept him or her.

All programs are guided by a set of competencies, which are common to all programs, promulgated by ACEND, the Accreditation Council for Education in Nutrition and Dietetics of the Academy of Nutrition and Dietetics, which is a gate keeper for the USDE (United States Department of Education). Each program determines appropriate learning activities for its own Program to allow its Interns to demonstrate the achievement of those competencies. 


The Internship provides a syllabus for each rotation, with learning activities delineated, making them achievable in a variety of settings. YOU do not have to come up with activities for Priority Nutrition Care Interns. The activities have been developed by experienced RDs who are very familiar with activities in each type of rotation.  The activities are generic enough that you may tailor them to meet not only the needs of the Interns but those of your site as well. There is a separate syllabus for each of the rotations, Clinical Nutrition, Food Service, Community, and the Elective and these are e-mailed, together with the Program Handbook to each preceptor prior to the Intern's arrival at the site. We do not provide syllabi in advance of an applicant being accepted into our program.


PLEASE NOTE: If you have already precepted one of our Interns and filled out our form, you do not need to complete the whole form again, assuming your contact information has not changed. Please fill in your name and that of the applicant or current Intern for whom you have agreed to precept, so that we will know that you have agreed to precept a different Intern. Our current form requests that you attach a resume and recent continuing education, as required by ACEND.  As such, if you have not sent us that information previously, we would appreciate it if you would do so now.  You could also email the information to

This is NOT an application to become a Preceptor. It is merely a form we use to collect information that we are required by our accrediting body, ACEND, to have on file for every preceptor to prove that Preceptors are qualified, including Registration numbers for RD/RDNs, resumes, and evidence of continuing education. We are required to provide this information when we complete a self study and have a site visit. As such, we are unable to place a student in a setting where that information is not shared with us.

GOOD NEWS ! CPEUs for Precepting 

The Commission on Dietetic Registration (CDR) started awarding continuing education credit for preceptors starting June 1, 2017 !  Precepting will be included under the current Leadership Activity Type with a three (3) CPEU/year, fifteen (15) CPEUS per five year recertification period maximum for Leadership and/or Precepting activities.
